Since 1998, Mayo Clinic's "Native CIRCLE" program has provided thousands of free education materials to programs. The funders tell us that the Native CIRCLE needs to improve the evaluation of the products and the program. The attached survey will take about 20 minutes of your time (more time if you add a lot of comments) and by completing it, you can help keep the Native CIRCLE funded and supported for many more years.

You may order up to 15 different items (including video/dvd), in quantities as follows:
Up to 50 copies of each print item1 or 2 copies of each video/dvd
